implementation of SBAR communication report. Moreover, there has been unsatisfactory patient outcomes which has become an issue of concern in the healthcare center.
Through the implementation of this tool, it is believed that there are going to be positive outcomes in hospitals.
The mode of communication that has been used over time fail to deliver clinical messages efficiently. As a result, this assessment proposes the use of SBAR

Some of the findings gathered from this study comprise of improvements after the implementation of patient’s survey items. Also, the study found out that after
implementation, there was an improvement at the nursing perceptions of report especially on patient safety and involvement in care.

Some of the evidence-based solutions to consider for this project are the use of different tools together with bedside shift report to see the most effective one. A
limitation that can be possible in this project is on participant sampling. It is possible that the sample that will be used may not represent the entire patient
population.
