This week, begin working on a comprehensive care plan for the aggregate

In addition, it should include a disaster management plan with the following components:

Phase 3: Analyzing the aggregate strengths and weaknesses
The strengths of the aggregate and the community living there are that despite previously being among the least healthiest cities, the aggregate has been making
significant efforts to improve its health through healthy schools and healthy community initiatives. One of the efforts is Spartanburg’s Way to Wellville that have
created a culture of health where people are ready to work together to promote health for all (Upstate Biz SC, 2016). The city and the entire Spartanburg County has
worked towards rallying community partners towards the achievement of a common objective of wellness. These attempts have worked well in significantly reducing teen
pregnancy, increased affordable houses, increased education opportunities, and access to medical facilities and healthful meals. Other strengths include enhanced
access to healthcare and social amenities for low-income earners and citizens of the county who cannot afford insurance policies and other creative tactics to
integrate behavioral wellness practices into the aggregate.
The strengths of the city and the community living in the aggregate are that it faces the health problems of high rates of children and child obesity, a high number
of people living with high blood pressure and diabetes, and risk of suffering from heart attack. The county also has high poverty rates, elevated levels of air
pollution, inadequate education facilities, poor living conditions along with other health problems.
Applying MAP-IT to this assessment, the first step is to mobilize, and there are already some active partners willing improve the health conditions of the city. The
next step is assessing the needs of the aggregate, which from the above weaknesses include making lifestyle modifications and improving the wellness conditions. The
partners then plan on the best way to make lifestyle changes and reduce the above lifestyle related diseases. The last step will be to track the progress by collecting
data to determine if the rates of the diseases have reduced .
Phase 4: Risk Assessment
The family, home, environment and risk assessment if the family was conducted by reviewing secondary data on the health risks facing families living in the City of
Spartanburg, interviewing the residents and through observations .
The selected family residing in the aggregate is made up of Dwight Rice, his wife Carey Rothschild and their three children aged 19, 15, and 11 years. The family
ethnic background is white, with middle-income economic status, and has lived in the city for more than a decade. The family’s recreational activities involve visiting
the parks in the city and the county. Dwight Rice has been living with diabetes for the last seven years. This condition has significantly affected the family
financially and sociologically. The family has incurred significant expenses in managing Rice’s condition and has required making several changes in their diet and way
of living. However, these changes have helped the other family members to adopt these lifestyle changes made to suit Rice as a strategy for preventing them from
suffering from diabetes or other lifestyle modifications. This approach may work well in the long-run to help other members to live healthy lives.
The family lives on the outskirts of the city in their single-family detached home that was built in the 1980’s. The family house has three bedrooms. The family has
the advantage of having a decent house unlike most of the families that live in some poor housing, which affects their lives negatively. The family home is safe for
the family and at the current time do not pose any risks to them.
The family’s environment poses several health hazards to them. The city has some industries and manufacturing plants and does not have effective zoning regulations and
land-use controls. Due to this state, the city has been affected by environmental exposures that expose the residents to health risks. For instance, the city has a
fertilizer plant, a former municipal landfill, a chemical plant and a textile mill. The selected family and other families live proximity to the environment exposures,
which expose them to high rates of bone, colon, and lung cancer, respiratory illnesses, elevated levels of infant and adult mortality, miscarriages and congenital
disabilities.
The aggregate also have poor transport infrastructure, inadequate sewer, and water services, inadequate medical facilities as the city have only three medical
centers, few economic opportunities, declining property values and public safety issues. All these issues expose the family to different risks. The inadequate sewer
and water services mean that the residents’ sewer is not disposed of in the proper way that exposes them to water-borne diseases. The lack of water services has in
some instances caused the family to lack clean water that exposes them to more health risks. The few economic activities affect the family in that the adults of the
family and other residents in the aggregate earn low incomes and do have opportunities to improve their income. As stated above, the family is in the category of
middle-income. The family cannot afford to pay for medical insurance, which limits the medical services that they can receive. This situation has been the reason why
the family has incurred significant amount to meet the medical costs of managing diabetes. The family faces the risk of suffering more economically in case another
member of the household suffers a long-term sickness .
