Go to the Arizona Board of Behavioral Health Examiners Website (http://www.azbbhe.us/). Then click Meeting Information, then scroll down to “Board Actions on or before 04/30/14.” From there, choose the year you are assigned to (see below), and download the report.

• If your first name starts with N-S, choose someone from 2012.

You will need to pick one provider who was brought before the board due to an ethical violation in the year you were assigned to. PLEASE DO NOT give the providers name. Please answer the following.
