• You are creating a Job Safety Analysis so that you can (hypothetically) use its output to create training materials, etc. for others to use, who would otherwise not be familiar with the hazards and their controls

1. Identify a job that interests you – one that you are familiar with – and choose one main task to analyze. Please note that if you choose a task that you are NOT familiar with, you may receive zero (0) on the assignment. (this can be a task you do at home or at work)

Provide an Introduction – what is the job and why did you choose this particular task to analyze?

Briefly overview the purpose of the task – what is it intended to accomplish (what work is being done)?

2. Using the table provided in the Word document“Assignment 4 Task Analysis Form”, break the task into individual steps (between 6 to 10 is just right). Remember a job contains many task and you are analyzing only one

Complete this Task Analysis Form, identifying the specific hazards, risk determinations for each hazard and control measures for each step. In the Hazards Column indicate which type of the four Hazards it is.

The PDF document “Assignment 4 JSA Risk Tables” is your source for risk determination data. Think of this as a road map where you use table one to calculate exposure and occurrence levels which then lets you determine probability in table 2. Table 3 lets you calculate the consequence which one combined with probability  which lets you determine risk in table 4. Please note: when proving risk course on your form you can use the numbers from table 5, you do not have to write high, medium or low.

3. On the same page as the Introduction, provide a Conclusion: By completing the assignment, what did you learn about this task and its hazards? What did you learn about the JSA process?

Your submitted assignment must contain a 1-page Word document with the Introduction & Conclusion, single spaced, 11 Arial font – with NO title page. The number of Task Analysis Form pages to be attached to the Intro/Conclusion page depends on the chosen number of individual steps, and the quantity of Hazards and related Controls identified for each step. There should be approximately 6 – 10 steps in the task.
