Question 1. 200 words. 4 marks

You are going to reflect on your own experience of undergoing cognitive screening, following the MOCA activity, as described in the Guide to Module 1. Please complete
the activity and then write a 200 word reflection about your experience as your answer to Question 1. In your reflection please:

focus on any factors you feel may have impacted your ability to perform at your best.
suggest ways in which these factors could be addressed in a real-life situation.

Question 2. 200 words. 4 marks (3 marks for content, 1 mark for correct referencing/in-text citations).

Irene and David are a married couple who have lived much of their adult lives in Hobart, Tasmania. David was diagnosed with Alzheimer’s disease some five years ago.
The diagnosis was confirmed by a brain scan and follow-up cognition tests, conducted by a geriatrician. Irene describes progressive changes in David’s cognition and
function, including being not able to tell the time, increasing forgetfulness, and gradual loss of ability to carry out activities of daily living. She describes
however that he still has moments of total clarity.

When David was diagnosed with Alzheimer’s disease several years ago, a range of early support measures may have helped both David and Irene as they adjusted to the
diagnosis. Using at least TWO sources of scholarly literature to support your response, please briefly discuss ONE early support measure recommended to help those
affected adjust to the uncertainty of dementia. (Hint: this module’s prescribed reading from the Downs and Bowers 2014 text will help you to get started). You will
need to provide in-text citations in your answer, and the sources you use will need to be presented in a reference list at the end of your work.
Question 3. 200 words. 4 marks

Briefly explain four reasons why it was important for Irene to be consulted, as a reliable informant, when David’s diagnosis of dementia was being made.
