Leon is a 55-year-old African American male. He’s a nonsmoker and nondrinker, and is a healthy weight for his height. He has a family history of Type 2 diabetes, and the last lipid panel he had was abnormal. On weekends, he coaches a youth baseball team in his community and loves to eat hot dogs and nachos with the children after they play. One month ago, he was seen in the emergency department for chest pain and dyspnea. He eventually had a cardiac catheterization which showed no evidence of coronary artery disease but he did have severe, uncontrolled hypertension. He was discharged on aspirin, thiazide diuretic, and an ACE Inhibitor. He is being seen for hospital follow-up.
